NEW PATIENT REGISTRATION PROCESS
Patients can register by either completing registrations forms in the practice or online. During Covid-19 (coronavirus) restrictions registration applications online are our preferred method.
To register with the practice you are required to complete the following TWO forms:-
1. Pre-registration Form (GMS1) which can be digitally signed and submitted.
AND
2. New Patient Questionnaire - Adult
or New Patient Questionnaire - Under 16
We do ask, if possible, that you provide photographic identification and/or proof of address for all adult registrations. Sight of each child's 'Red Book' is also helpful at registration so we can ensure information collected at previous developmental checks is transferred to the GP record.
Registration will not normally be declined should identification documents not be available. Proof of ID documents and new patient registration health questionnaires can be emailed to our admin email address: somccg.admin.redgatemc@nhs.net . Once your registration application has been processed emailed ID documents will be destroyed.
