Birthday Annual Review

From April 2014 we will be changing the way we manage patients with long term health conditions.      Patients who have conditions such as diabetes, asthma, COPD, hypothyroidism, cardiovascular disease or high blood pressure will have a single review for all their long term medical problems each year in the month of their birthday .  This will reduce the number of visits patients have to make to have their long term conditions monitored which we hope will be much more convenient.  We would encourage you to contact the practice around the time of your birthday to book a Birthday Review  appointment if this applies to you.

Transferring from the old system to a birthday review system is going to be challenging and may result in some patients being seen earlier or later than they initially expected.  The doctors would like to thank all their patients for their support whilst we are implementing these changes

 

Appointments

docbabyWe are able to offer a comprehensive range of appointments. All appointments can be booked by ringing our main switchboard number on 01278 454560. By providing the receptionist with adequate information, they will then be able to offer you an appointment with the most appropriate clinician.

The Duty Team are available from 8 am to 6.30 pm. If you require an urgent appointment with a GP or Nurse Practitioner and it needs to be dealt with sooner than the next routine appointment, you will be asked to provide a brief description of your problem by the receptionist and a telephone number that you can be contacted on between given times. A Duty Team clinician will call you back during a pre-arranged time slot and agree the most appropriate course of action with you – this may be an appointment later in the day with a GP or Nurse Practitioner, you may be offered an appointment for a blood test followed by an appointment with a GP a few days later, or the matter may be sorted over the phone without the need for you to see a clinician. The aim of the Duty Team Surgery is to ensure patients are seen by the most appropriate clinician

Routine Appointments  If your condition is non-urgent, you have the option to book a routine appointment up to 6 weeks in advance with a Doctor of your choice if this is more convenient for you.

Nurses based in our practice treat patients for a wide range of common conditions. You can expect to see a nurse within one working day and all of them are qualified to prescribe from a range of medicines.

Let us know if more than one person in the family needs to be seen. We can give you a longer appointment if necessary. Tell us if you want someone to accompany you during an examination or a private room to discuss any matters.


How do I make an appointment?

When making an appointment please give the receptionist your name, address and any details that are relevant for the appointment. The receptionist might ask some general details about the problem. This is in order to give you the most appropriate type of appointment with the right person.

If blood or other tests are required, please tell the receptionist as we need to ensure these samples are taken in the morning to enable them to go to the hospital for processing the same dayl

  • Smear Test
  • Asthma Check
  • Diabetes Check
  • Doppler Test
  • Immunisations
  • ECG Test
  • IUD Check
  • Travel Health
