Birthday Annual Review

From April 2014 we will be changing the way we manage patients with long term health conditions.      Patients who have conditions such as diabetes, asthma, COPD, hypothyroidism, cardiovascular disease or high blood pressure will have a single review for all their long term medical problems each year in the month of their birthday .  This will reduce the number of visits patients have to make to have their long term conditions monitored which we hope will be much more convenient.  We would encourage you to contact the practice around the time of your birthday to book a Birthday Review  appointment if this applies to you.

Transferring from the old system to a birthday review system is going to be challenging and may result in some patients being seen earlier or later than they initially expected.  The doctors would like to thank all their patients for their support whilst we are implementing these changes

 

Health Visitors

Home visits are made according to family and individual needs. A home visit is made around the eleventh day after the birth of a new baby and the Health Visitor will undertake a Newborn Hearing Screening Programme.

Routine appointments with Health Visitors:

Ante NatalHealth Promotion visiting from 28 weeks of pregnancy
Primary VisitAfter you are discharged by the midwife, home visit within 14 days of birth
6-8 weeksBaby assessment
1 year1 year review
2-2½ years2 year review

The Health Visitors are also involved in activities for all age groups. These include community health programmes, e.g. smoking, breast feeding awareness campaigns, parentcraft, group work teaching on all aspects of health, child protection and training of other professionals.

The Health Visiting team are at the Child Health Clinic every week for advice and support, as well as a good opportunity to meet other parents.

Health visiting hours are 9am to 5pm Monday to Friday.  If they are unable to take your telephone call, please leave a message their answerphone and they will get back to you but this may not be on the same day.  If you are worried about the health of your child either contact your GP or NHS 111

Wednesdays - Willows Children's Centre 10.30 am to 11.30 am
